“Hi, I'm Mary Rivera Founder and Owner of Lucid Counseling and Wellness Consultants. We are a Private Practice Entity since 2018. I am also a Licensed Professional Counselor-Supervisor#75057 (LPC-S) and Licensed Chemical Dependency Counselor#15254 (LCDC) in the state of Texas. Lucid Counseling and Wellness Consultants is committed to providing a comprehensive approach to mental health wellness, brain health, and physical health. Our team of experts is dedicated to helping individuals achieve overall well-being through personalized counseling and wellness programs that cater to the physical, emotional, mental, social, spiritual, and financial aspects of their lives. We believe that addressing the whole person in mind-body-spirit can bring out the healthiest, happiest version of ourselves, and we strive to help our clients achieve this state of.  It is of utmost importance to take care of our entire being, encompassing our physical, emotional, mental, social, spiritual, and financial health. By paying attention to all these aspects, we can achieve a state of complete well-being, leading to a happier and healthier version of ourselves. Therefore, it is essential to prioritize our holistic health and strive towards living our best lives. ACCESS TO YOUR HEALTH RECORDS: Clients have the right to request copies of their health care records. HOW TO SUBMIT A REQUEST: Requests may be made by emailing our administrative office at lucidcounselingandwellness@gmail.com or by sending a secure message through the Client Portal. HOW RECORDS ARE DELIVERED: To protect confidentiality and comply with HIPAA regulations, records are provided electronically through the secure client portal. FEES & TIMELINE: An administrative fee of $75 may apply for full record requests to cover preparation and secure delivery. Records are typically released within 15 business days of receiving a written request and all required consents. COMPLAINTS, CONCERNS, QUESTIONS: We are committed to providing services grounded in professionalism, respect, and care. If you have concerns about your experience, we encourage open communication.
